Urea is an organic compound that, when used topically, can have, depending on the concentration of use, a hydrating or keratolytic effect. The hydrating effect is obtained using low concentrations and is due to an increase in water binding sites at the level of the stratum corneum and a reduction in trans-epidermal water dispersion. The kerato-reducing and keratolytic effect is obtained at higher concentrations (30-50%) and the mechanism seems to be the alteration, through breaking of hydrogen bonds, of the quaternary structure of the keratins of the stratum corneum.
