Description
Nasal spray indicated for purulent infectious rhinosinusitis (acute and chronic) to promote the evacuation of mucopurulent secretions from the nasal sinuses and ear tubes, decongest the nasal sinus mucosa, and reactivate mucociliary clearance. It is useful in the treatment of purulent infectious rhinosinusitis and/or effusion of otitis media when there is discharge from the nose or throat of thick mucus (yellow, greenish, or white), and/or craniofacial pain (affecting one or more paranasal sinuses), and/or hyposmia (reduced sense of smell), in association with one or more of the following symptoms:
- nasal-sinus congestion (especially if unilateral);
- loss of smell (anosmia);
- hearing loss (hypoacusis);
- nosebleed.
The solution containing Cucurbitacine B-D4-E acts on the surface of the mucosal epithelium causing electrolytic and osmotic alterations which promote:
- the expulsion of mucopurulent secretions;
- restoration of ciliary motility;
- reduction of mucosal edema;
- restoration of pneumatization of the paranasal sinuses (air pressure in the paranasal sinuses).
Furthermore, BDIE Cucurbitacins produce a surfactant effect on mucopurulent secretions, promoting their dilution and fluidification.

How to use
For adults (>12 years)
Nasal Spray: 1 or 2 sprays per nostril, once or twice daily (morning and evening), for a course of 7 consecutive days. The 7-day treatment may be extended up to a maximum of 14 days or repeated after a 6-7 day interval without treatment, based on clinical response and symptoms.
The dosage should be adjusted based on the tolerability of the product, the severity of the symptoms and the quantity of mucopurulent secretions present in the nasal-sinus area and/or auditory tubes.
The duration of treatment can be adapted based on clinical response and symptoms, however, the minimum duration of treatment is seven days: shorter treatments do not guarantee a tangible effect.
For chronic rhinosinusitis (even latent) or in the post-surgery period, it is recommended to carry out at least two cycles as indicated above, always spaced by at least 6-7 days without treatment.
For nasal spray (children and adults)
1) Blow your nose to remove any mucus or debris to allow the product to effectively reach the nasal areas.
2) Remove the dispenser safety cap and protective cap.
3) Prime the pump by pressing 2-3 times until you obtain a regular nebulization.
4) Gently insert the dispenser into one nostril, keeping your head upright and tilted slightly forward.
5) Press the spray once while slowly inhaling through your nose.
6) Repeat in the other nostril following the same procedure.
Do not exhale or blow your nose immediately after spraying to avoid dispersing the product (wait 10-15 minutes). Use on children must be supervised or assisted by an adult.

Components
Active ingredients: BDIE cucurbitacins extracted from Ecballium elaterium. The amount present in each formulation is indicated on the individual product bottles.
Excipients: purified water, ethyl alcohol, mint flavouring, eucalyptol, methyl parahydroxybenzoate, propyl parahydroxybenzoate, polysorbate 80, sodium chloride, monosodium sodium phosphate dihydrate (or citric acid monohydrate), disodium sodium phosphate dodecahydrate (or sodium citrate dihydrate).

Warnings
- All administration operations must be performed delicately, avoiding inserting the tip of the dispenser too deeply into the nostrils and maintaining an upright position during use.
- Close the bottle after each application.
- Store the product away from light and direct heat sources.
- Do not use the product for more than 14 consecutive days without consulting a doctor.
- For patients with nasal vascular fragility, vasomotor rhinopathy (allergic or non-allergic cell-mediated), or in the presence of chronic inflammation or post-surgery, it is recommended to start with a single spray per nostril per day. If well tolerated, after 2-3 days of treatment, the dose can be increased to two sprays per nostril, twice daily (standard dosage).
- If burning, irritation, frequent sneezing, or watery eyes occur, reduce the dosage to one spray per nostril per day; the burning sensation generally disappears within a few minutes.
- CILIACELL may cause burning if applied to damaged or sensitized mucosa, for example in the presence of chronic inflammation or prolonged use of decongestant/vasoconstrictor drugs.
- For children, use must be carried out under adult supervision.
Contraindications:
- hypersensitivity (allergy) to Cucurbitacins BDIE or other components of the product;
- untreated viral or bacterial upper respiratory tract infections;
- acute otitis media with effusion without medical supervision.; - Use in pregnant women and children under the indicated age or in the presence of conditions that require specific medical evaluation.
The most common effects are:
- rhinorrhea (evacuation of fluids mixed with clear, yellow-green or white mucus), which promotes decongestion and the restoration of patency of the upper respiratory tract.
It may also occur:
- temporary congestion of the nasal mucosa with sensation of a blocked nose;
- intensification of symptoms during the first days of treatment (cranial facial pain);
- burning in the nostrils immediately after administration;
- sneezing and watery eyes.
The rarer effects are:
- irritation of the throat and palate;
- small streaks of blood in the evacuated mucus (epistaxis), especially in subjects with nasal vascular fragility.
- mild fever (only for a few hours during the first few days of treatment).
Overdosing increases the likelihood of the side effects described. If persistent or severe side effects occur, discontinue use and consult your doctor or pharmacist.
All operations must be performed delicately: avoid inserting the tip of the dispenser too deeply into the nostrils and maintain an upright position during application.
After each administration:
- close the bottle tightly;
- do not expose to direct light;
- keep away from heat sources.
If you experience severe burning, persistent irritation, frequent sneezing or watery eyes: reduce to one spray per nostril per day or temporarily discontinue use - these effects are generally temporary.
Specific warnings:
- overdose is absolutely not recommended;
- use in association with sympathomimetic drugs is not recommended;
- The aerosol therapy solution must be administered exclusively through a micronized nasal douche. Do not use nebulizers with masks or mouthpieces specifically designed to treat the lower respiratory tract;
- do not use the product after the expiry date;
- the CILIACELL solution is not sterile and is not injectable;
- keep the product out of the reach and sight of children;
- do not use the product if the bottle is not intact and well sealed.

Conservation
Store between 10°C and 25°C.
Shelf life in intact packaging: 24 months.
Shelf life after opening: 2 months.
