Pterostilbene is a dimethylated derivative of resveratrol that is naturally present in several plants including blueberry. Compared to resveratrol, it is more bioavailable and therefore more easily absorbed. A study has shown that compared to resveratrol, at equivalent doses, pterostilbene is a much more potent modulator of cellular stress than its demethylated counterpart. From a molecular point of view, pterostilbene, like all stilbenes, contributes to the activation of the cAMP signaling pathway by inhibiting cAMP phosphodiesterase.

From a molecular point of view, pterostilbene reduces the expression of gluconeogenesis enzymes such as glucose-6-phosphatase and fructose-1, 6-biphosphatase, while increasing the expression of glycolytic hexokinase. In addition to mitigating hyperglycemia, pterostilbene has been shown in vitro and in vivo to act on lipid metabolism with anti-lipogenic, anti-obesity, and hypocholesterolemic effects. In 3T3-L1 preadipocytes, pterostilbene treatment reduced adipocyte population growth, adipogenesis, and thiacrylglycerol accumulation, altered PPAR-? receptor gene expression, resistin, and fatty acid synthase (FAS) expression, increased adiponectin levels, and decreased leptin levels, resulting in a reduction in BMI in treated animals compared to controls. Other studies in hypercholesterolemic animal models fed oral pterostilbene 20 ppm demonstrated a 29% reduction in plasma low-density lipoprotein (LDL) cholesterol, a 7% increase in high-density lipoprotein (HDL) cholesterol, and a 14% decrease in plasma glucose levels compared to controls. The authors also found that in vitro pterostilbene increased PPAR-? activation in rat liver cells that was significantly greater than the amount of PPAR-? activation in rat liver cells. produced by the lipid-reducing agent clofibrate, used for the treatment of hypertriglyceridemia.
